End of Year Medication Pick-Up
 
Please pick up any unused medication(s) that your child has in the school health office by the end of the school year. The school cannot store medication in the office over the summer and unclaimed medications will be disposed.
 
As a reminder for next year:
  1. All nonprescription medication must be in the original bottle/package, unopened. All prescription medication must be in the original pharmacy container with correct information on the label.
  2. All medication, prescription and nonprescription, must be accompanied by a “Medication Consent” form with parent/guardian’s signature. Any changes in dose, time or medication will require a new form and a new container with the corrected pharmacy label.
  3. The “Medication Consent” form must be renewed each year.
