Parents, do you need to register your student at one of the Crane District schools? You'll need to stop by the school's office and complete the complete registration packet; however, for your convenience, an Enrollment Form is available for you to complete before reaching the school at which you'll be enrolling your child. The enrollment packet which you will be required to complete at the school contains forms requiring information such as:
- Home Address
- Home or Work Phone
- Parent's Names
- Emergency Contact Information
- Home Language Survey Form
- Immunization records
- Special Education needs
You must also supply the following documents (in person). We'll make copies and return the originals to you:
- Your student's birth certificate
- Immunization Record
- Report Card
- Withdrawal Slip
- Proof of Residency (valid Arizona driver's license, valid motor vehicle registration, valid U.S. passport, property deed or mortgage documents, residential agreement or lease, utility bill (water, electric, gas, cable, phone), bank or credit card statement, W-2 wage statement, payroll stub, Certificate of Tribal enrollment or other ID issued by recognized tribe, or other documentation from a state or federal government agency (Social Security Administration, Veterans' Administration, Arizona Department of Economic Security, etc.)
